I fully assume all risks that I can be injured or harmed while in or about the premises of Enso MMA. I understand that Enso MMA are not medically supervised facilities and the owner, employees, trainers, and members are not qualified to assess my physical fitness for any purpose nor to render emergency medical assistance. I understand that due to the 24 hour access nature of Enso MMA there may be times when I am at the facility alone, with no one to assist me, render aid, or summon emergency personnel.

I attest that I am in good physical condition and I have no disability, impairment or medical condition which will prove detrimental when I participate in the physical activity which I have requested. Enso MMA urges all members to obtain a medical examination prior to the use of any exercise equipment.

I agree to fully release, discharge, indemnify and hold harmless Enso MMA, its owners, employees, instructors, agents or affiliates (“Releasees”) from any and all present and future claims or demands resulting from injury, death, or in property damage, whether caused by active or passive negligence of Releases or otherwise, while I am in or about the Enso MMA premises or any facilities or equipment therein.
